This represents a year-on-year growth of 21.70% compared to sales of $2.505 billion in the corresponding quarter of the previous year. Segment and Operational Details
Instruments and accessories revenue totaled $1.80 billion, up 18%. Systems revenue came in at $769 million, up from $590 million in the comparable quarter last year. Worldwide procedures grew by approximately 15% year-over-year. Intuitive Surgical placed 546 da Vinci surgical systems in the quarter, versus 427 in the prior year’s quarter. The company’s da Vinci surgical system installed base grew to 12,094 systems, up 12% year-over-year.
The Ion endoluminal system also saw significant growth, with its installed base reaching 1,152 systems, an increase of 21% compared with 954 as of September 30, 2025. Ion procedures grew approximately 31% year-over-year.
"Our strong performance this quarter was marked by broad-based adoption across our multi-port, single-port, and Ion platforms, with customers using our systems to perform, on average, over 10,000 procedures per day throughout the period," said Dave Rosa, CEO of Intuitive Surgical.
The company ended the period with $6.80 billion in cash, cash equivalents and investments, a decrease of $1.83 billion during the quarter, primarily driven by cash used for repurchases of common stock and purchases of property, plant, and equipment. In the third quarter, the company repurchased 7.8 million shares of its common stock for $2.94 billion.
Strategic Pricing and Guidance Updates
During the earnings call, management announced strategic pricing actions aimed at expanding adoption in high-volume benign procedures. These actions are expected to reduce da Vinci instruments and accessories (I&A) revenue per procedure by approximately 3% to 5% on a mix-constant basis once fully implemented. The changes include extending the useful lives of certain EndoWrist instruments and introducing volume-based pricing, beginning in the first half of 2027. Additionally, policy-driven price reductions in China will take effect starting November.
For full-year 2026, Intuitive Surgical expects worldwide da Vinci procedures to increase approximately 14% to 15%, narrowing the previous guidance range of 13.5% to 15.5%. Gross profit margin is expected to be between 68.7% and 69.3% of revenue. Non-GAAP operating expense growth is expected to be 11% to 13%. The company also updated its non-GAAP other income forecast to between $300 million and $310 million, reflecting lower average cash balances following share repurchases.
What the Numbers Show
The divergence between the 25% growth in adjusted EPS and the 21.70% growth in revenue suggests an expansion in profitability or operational efficiency during the quarter. However, GAAP net income attributable to Intuitive Surgical fell to $523 million ($1.49 per diluted share) from $704 million ($1.95 per diluted share) in the prior year quarter. This decline was driven by a pre-tax charge of $450 million related to the settlement of previously disclosed antitrust class action litigation. The settlement, reached in October 2026 without admission of liability, resulted in a net-of-tax expense of $347 million, or $0.99 per diluted share.
Additionally, both GAAP and non-GAAP net income included a benefit of $22 million, or $0.06 per diluted share, related to refunds for tariffs paid in prior periods under the International Emergency Economic Powers Act (IEEPA). Despite the strong operational metrics and a beat on adjusted top and bottom lines, ISRG shares were down 10.50% in after-hours trading, trading at $392 at the time of publication.
